New IRB Form for Key Study Personnel Changes

Need to revise the Key Study Personnel sections of your IRB application? The IRB has created a new “Personnel Changes” submission form and it is now available in iRIS.

When do I use the Personnel Changes form?

Use this form when the ONLY changes you are making involve adding or removing Key Study Personnel. 

Who are Key Study Personnel?

UCSF Key Study Personnel include the Principal Investigator, other investigators and research personnel who are directly involved in conducting research with study participants or who are directly involved in using study participants’ identifiable private information during the course of the research. Key Personnel also include faculty mentors/advisors who provide direct oversight to Postdoctoral Fellows, Residents and Clinical Fellows serving as PI on the IRB application.

How do I use the Personnel Changes form?

As you fill out the form, read the guidance within the form to make sure you’re answering the questions as accurately as possible.
