Updated HIPAA Research Authorization Form (2026) | Human Research Protection Program (HRPP)

The University of California Office of the President (UCOP) has released a revised HIPAA Research Authorization Form, rewritten in plain language to make it easier for research participants to read and understand.

What's changing:	The revised HIPAA Research Authorization Form is now the approved form for use. Translated versions and UCOP's user guide/instructions for using the revised form are available. Beginning October 1, 2026, the revised form is required for new submissions; the previous version should no longer be used for new submissions after October 1, 2026. The UCSF IRB HIPAA page has also been updated with the revised forms and guidance.
What you need to do:	Use the revised form for all new study submissions on or after October 1, 2026. Already-approved studies don't need to submit a modification solely to replace the HIPAA authorization form, and use of the revised form remains optional for them. However, if you're submitting a modification for another reason, we encourage you to incorporate the revised form at that time. Do not obtain a new HIPAA authorization from participants who have already documented their authorization. Their existing authorization remains valid.
